Human Resource Procedural Requirements:
1. Completed Health Assessment Form- A health examination for the past 6 months before the projected start date;
2. Completed Department of Justice and FBI Background Check (Live Scan);
3. Employee’s Proof of Auto Insurance (If driving of agency’s vehicle is required);
4. Completed Department of Motor Vehicles Background Check (If driving of agency’s vehicle is required);
5. Proof of eligibility to work in the USA.
