The Care Manager provides supervision for Big Care Team Assistants generating and reviewing performance data and devising and applying improvement strategies. The Care Manager supports the patient-centered medical home by working with patients, families, providers and employees to promote timely access to needed care, daily continuity of care coordination and coaching patients, families and caregivers to understand the patient’s care plan and self-care management responsibilities. There are a Big Care Team and small Care Teams. The Care Manager is responsible for supervising staff who belong to the Big Care Team while also working with the Assistant Nurse Manager to review the performance of Small Care Teams. Care Team at APHCV is an evolving process, influenced by the rapidly changing health environment, and the Care Manager shall be open minded and flexible working in such environment.
APHCV expects all employees to respond and participate to emergency per emergency policies and procedures.
DUTIES AND RESPONSIBILITIES:
Work with Nurse Manager and Care Team Workgroup, which develops annual goals and performance measures for Care Team structure. Implement the goals and meet the performance expectation. Monitor progress throughout the year.
Develop and implement Quality Improvement (QI) and performance improvement strategies. Apply the principles of comprehensive, community-based, patient-centered, developmentally appropriate, and culturally and linguistically sensitive care coordination services. Develop policy and procedures. Review data and generate reports.
Hire, supervise and evaluate the Big Care Team assistants
Current LVN license in the State of California or Bachler or Master Degree in health science.
Recent experience in a clinic or outpatient setting
Proficiency in various data generating software including electronic health record.
BLS certified according to the AHA guidelines if LVN.
Ability to supervise
Ability to review and analyze data and structure/generate reports.
Good communication and interpersonal skills, ability to think critically and to write document such as policy and procedures.
Ability to demonstrate patience and convey a warm, caring attitude toward patients and staff,
Willingness to learn new skills,
Ability to work with people of diverse cultural, educational, social, economic, and linguistic backgrounds,
Regular predictable attendance during clinic hours
Ability to work efficiently and accurately with minimal supervision
Ability to teach others
