Quality Improvement Nurse

Position : Quality Improvement Nurse
Status : Full-time; Non-exempt
Salary : TBD

DUTIES AND RESPONSIBILITIES

 Quality Improvement:

  • Annual confirm the QI measures with CEO and Medical Director for internal and external reports
  • Develop performance improvement measurement targets for quality, service and efficiency
  • Maintain and monitors documentation of all QI Activities
  • Ability to access and generate reports from systems such as i2i program
  • Ensures that improvement activities are documented and reported
  • Conduct a meeting and record meeting minutes for the Utilization Committee
  • Work with health plans and IPA regarding medication therapy management program, individualized care plan and address potential gaps in APHCV’s patient care
  • Conduct internal audits to achieve the goal per APHCV’s policy and procedures
  • Presents results of selected performance measures to defined organizational committees according to the APHCV’s policies and procedures
  • Analyzes data for patterns and trends in care delivery, discovering the root causes for specific patient care trends
  • Supervises Care Team Coordinator, Breast and Cervical Health Case Manager, and CPSP and Health Education Coordinator. Set up their annual goals for quality of care.
  • Supports in aggregating data and working with auditors to develop corrective action plans

Quality Assurance:

  • Assists Medical Director in conducting peer review per APHCV Peer Review Policy and Procedure
  • Ensures all findings and improvement activities identified through QI work and internal/external audits

Patient Centered Medical Home:

  • Oversee PCMH in terms of quality of care
  • Conduct or facilitate training of staff in the quality improvement processes
  • Ensure that Care Coordinator effectively guide Care Teams by helping to measure quality and identify, refine, and implement performance improvements
  • Conducts Quality Improvement processes that includes regular review of performance data and evaluation of performance
  • Participate in population management activities

Improvement Education and Training:

  • Provides staff training on quality improvement processes, and provide documentation to Human Resources Department

Other Functions:

  • Oversee a merger of patient records
  • Attends APHCV assigned/external meetings and reports relevant information to staff
  • Assumes responsibility for own learning needs and communicates needs to supervisor
  • Participates in agency wide events as mandated by APHCV’s policies and procedures

REQUIREMENTS:

  • At least 2 years of staff supervision experience
  • Ability to apply, teach, and skillfully use techniques for system design, quality improvement, outcomes measurement, and statistical analysis
  • Ability to use electronic medical records, relationship data base, understanding of federal requirement and meaningful use requirements
  • Demonstrates the ability to identify quality improvement, develop solutions, exercise leadership, conflict management, and team building skills
  • Understanding of Patient Centered Medical Home
  • Superb written and oral communication skills with particular emphasis on presentations
  • Ability to work independently and performs under minimal supervision
  • Proficiency in Microsoft Office applications

EDUCATION/LICENSURE/CERTIFICATIONS:

Required:

  • Current RN license in the State of California
  • Bachelor’s Degree in Nursing

Preferred:

  • Master’s Degree in Nursing
