Community Health Quality Recognition Badges

APHCV is consistently recognized by HRSA for its achievements in quality improvement in the areas of access to care, health equity, health information technology, and quality of care. Below are all the Quality Recognition Badges that have been awarded in recent years.

 

Health Center Program Quality Leader Gold 2025, Health Center Program Preventive Health 2025, and Health Center Program Advancing HIT for Quality 2025.

National Quality Leader 2025 for Behavioral Health, National Quality Leader 2025 for Cancer Screening, and National Quality Leader 2025 for Diabetes Health

Health Center Quality Leader, Gold: Health centers receive Health Center Quality Leader Gold recognition if they are among the top 10% in CQM performance based on average Adjusted Quartile Rankings (AQR).

Preventive Health: Health centers receive Preventive Health recognition if they meet or exceed five of ten measure targets including the following: 76.9% target for dental sealants for children ages 6 to 9 years, 85.1% target for weight assessment and counseling for nutrition and physical activity for children and adolescents, 28.6% target for childhood lead test screening (ages 9 to 72 months), 20.8% target for childhood development screenings and evaluations, 85.8% target for screening for depression and follow-up plan, 80.0% target for tobacco use screening and cessation intervention, 86.0% target for adult Body Mass Index (BMI) screening and follow-up plan, 80.3% target for breast cancer screening, 79.2% target for cervical cancer screening, and 72.8% target for colorectal cancer screening.

Advancing HIT for Quality: Health centers receive Advancing Health Information Technology (HIT) for Quality recognition if they offer telehealth services, exchange clinical information online with key providers’ health care settings, engage patients through HIT, and collect data on patient health related needs.

National Quality Leader, Behavioral Health: To earn NQL Behavioral Health recognition, a Health Center must meet or exceed two of three measure targets: 18% target for Depression remission at twelve months, 85.8% target for Screening for Depression and follow-up plan, and at least 5% target for Screening, Brief Intervention, and Referral to Treatment (SBIRT) (Proportion of all patients receiving SBIRT).

National Quality Leader, Cancer Screening:  To earn NQL Cancer Screening recognition, a Health Center must meet or exceed at least one of three cancer screening targets: 80.3% target for Breast Cancer Screening, 79.2% target for Cervical Cancer Screening, and 72.8% target for Colorectal Cancer Screening.

National Quality Leader, Diabetes Health: To earn NQL Diabetes Health recognition, a Health Center must meet or exceed two of three measure targets: 86% reached for Adult Body Mass Index (BMI) screening and follow-up plan, 85% reached for weight assessment and counseling for nutrition and physical activity for children and adolescents, and 11.6% or lower for Diabetes: Hemoglobin A1c (HbA1c) Poor Control (>9.0%) – Inverse Measure (uncontrolled diabetes).

Patient-Centered Medical Home

APHCV has earned recognition as a Patient-Centered Medical Home since 2017 for its patient-centered approach to care consisting of a team-based care model that helps improve communication and coordination among providers, and has been shown to improve the patient experience and patient health outcomes. At APHCV, we are dedicated to providing the highest quality of care for our patients by providing coordinated, whole-person care that is patient-centered with accessible hours and locations.

Care’s Annual Provider Recognition Awards.  In April 2024, APHCV was recognized for having achieved Top Performing Clinic for 6 years in 2018, 2019, 2020, 2022, and 2023.  APHCV was also awarded for Top Performing Clinic in Chronic Care, and Excellence in Women’s Health and Pediatric Services for 2022.

APHCV was recognized by L.A. Care in 2024 for its Honor Roll in which top 20% of Providers in L.A. Care’s Pay-for-Performance (P4P) program are recognized for outstanding service in patients care.

APHCV was also recognized by L.A. Care for its excellence in Comprehensive Diabetes Care, Pediatric, and Adult Services in 2018.

County of Los Angeles Quality and Productivity Commission

Special Merit Award

APHCV, along with the Department of Public Health, Department of Health Services, Eat SF: Vouchers4Veggies, and other contracted Community Health Centers were recognized for its Food as Medicine: Prescribing Produce for Health program.  The Produce Prescription Program (PPP) addresses barriers to access by giving patients a specified amount of money to purchase fresh produce at participating local grocery stores.  The program enables healthcare provider to “prescribe” produce as part of patient care plans, empowering patients to manage their health through improved nutrition.  In addition to the Produce Prescription Program, APHCV’s Community Programs have provided weekly free produce pick-ups, food insecurity screening & referral, nutrition education, food and cooking demonstrations, and physical activity workshops to patients in need of food security and nutrition support since 2000. 

County of Los Angeles 36th Annual Productivity and Quality Awards Program

Silver Eagle Award

APHCV, along with the Department of Public Health, Department of Public and Social Services, and other contracted entities, received recognition in 2023 for The CHOI Story: Healthcare Navigation for Families program. This award recognizes the efforts of APHCV and other contracted partners across Los Angeles County that outreach and assist individuals and families in need of public health insurance, case manage those clients to ensure enrollment and successful use of their health insurance, and assist with renewals to ensure that they maintain their health coverage.  Since 2003, APHCV’s Enrollment Services Program has assisted patients and clients in health insurance enrollment and case management.


American Heart Association (AHA) & American Medical Association (AMA)

American Heart Association Target: Type 2 Diabetes Gold 2025, American Heart Association & American Medical Association Target: BP Gold + 2025, and American Heart Association Check, Change, and Control Cholesterol Gold 2025.

APHCV has been recognized by the American Heart Association’s Target: Type 2 Diabetes™ Gold Award for its commitment to addressing the relationship between diabetes and the risk of heart disease and stroke.  APHCV was also recognized in 2021, 2023, and 2024 at the Gold recognition level.

APHCV has been recognized by the American Medical Association and American Heart Association for its commitment to improving blood pressure (BP) control rates, earning Gold Plus recognition as part of the Target: BP™ initiative.  APHCV was also recognized for this initiative in 2023 and 2024, earning Gold Plus recognition.

APHCV has also been recognized by the American Heart Association with a Check. Change. Control. Cholesterol™ Gold Award for its commitment to reducing the risk of heart disease and stroke through improved cholesterol management.  APHCV was also recognized for this award in 2023 and 2024.
