John Hoh, M.D. | Medical Director

Dr. John Hoh is APHCV’s Medical Director specializing in Internal Medicine with added formal training in Geriatric Medicine. With APHCV since 1996, he received his medical degree from Temple University in 1983. He completed a 3-year medical residency at Montefiore Medical Center and a 3-year fellowship at John Hopkins Medical Center and the National Institute of Health-Gerontology Research Center. Dr. Hoh treats patients in English and Cantonese.

Lena Al-Sarraf, D.O.

Board certified in general practice medicine, Dr. Al-Sarraf completed her medical degree at Western University and completed additional post-doctoral training at USC. She is fluent in English and Arabic. She can also discuss medical issues in Spanish. She enjoys reading, biking, sports, and music. Dr. Al-Sarraf practices at Belmont Health Center.

Wei Min Chan, PA-C

Min is a Florida native who attended Nova Southeastern University where she received her Masters in Health Sciences. She is fluent in both English and Cantonese. She enjoys exploring her new California home with her dog and trying all sorts of new foods. Min practices at Los Feliz Health Center.

Jing Deng, M.D.

Dr. Deng is bilingual in Mandarin and completed her medical education at the University of Illinois at Chicago and Bengbu Medical College in China. Her specialty is Internal Medicine and completed her residency at the Wright Center for Graduate Medical Education in Pennsylvania. Dr. Deng enjoys cooking, reading, traveling, and spending time with family. Dr. Deng practices at El Monte Health Center.

Raymond Goei, D.O.

Specializing in Family Medicine, Dr. Goei received his medical degree from the Western University of Health Science/College of Osteopathic Medicine of the Pacific. He also received certification by the American Board of Family Medicine and completed his residency at San Bernardino County Medical Center. Dr. Goei is interested in working with computers and enjoys websurfing, home improvement, gardening and skiing. Dr. Goei sees patients at El Monte Health Center.

Robert Howard, Licensed Clinical Social Worker

Robert Howard grew up in Oklahoma and attended Boston University where he earned his Master of Social Work degree. Robert previously taught at Boston Children’s Hospital. He also has experience in working with children with various health care needs. Robert’s innovative work helps his patients work toward wellness. In his leisure time, Robert enjoys outdoor activities, music, concerts, and spending time with family. Robert sees patients at Los Feliz Health Center.

Kevin Ko, M.D.

Dr. Ko joined APHCV after seven years of service in outpatients and hospitalist care.  Dr. Ko’s education includes Family Medicine Residency at the University of Texas Southwestern (2014) with work at St. Paul University Hospital and Parkland Memorial Hospital, Dallas, Texas.  Dr. Ko enjoys traveling, working in orphanage care in Uganda and Haiti, and community development in Peru. He is fluent in Mandarin and also some Medical Spanish.

Dominic Koh, MSN, CPNP, ANP-C

Nurse practitioner and APHCV’s Director of Nursing, Dominic Koh has been with APHCV since 1997. He completed his nursing degree and a post master’s certificate at Cal State University, Los Angeles. He is a Certified Pediatric Nurse Practitioner (CPNP) and an Adult Nurse Practitioner – Certified (ANP-C). Fluent in English, Mandarin, and Cantonese, Dominic also enjoys traveling and the theater. Dominic sees patients at Los Feliz Health Center.

David La, D.O.

Dr. La was born in San Jose, CA. He graduated from Touro University in Nevada where he got his Doctor of Osteopathic Medicine in 2017. He completed his undergraduate studies in Psychobiology at UCLA, and a residency in Pediatrics at LA County/USC. He is licensed in the American Board of Pediatrics and California Medical Board. He has active certifications in Pediatric Advanced Life Support (PALS) and Neonatal Resuscitation Program (NRP). He speaks English, Vietnamese, and Medical Spanish. Dr. La sees patients at Los Feliz Health Center.

Melanie Lapeyrolerie, L.C.S.W.

Melanie has extensive experience in clinical social work and mental health services.  She received her Masters Degree in Social Work in 2006 from the University of Southern California and has been a Licensed Clinical Social Worker since 2013.  She is passionate in providing care to improve the lives of underserved individuals, families, and youth, including those with severe mental health, substance abuse, and other related behavioral and/or psychological challenges.  Melanie sees patients at Belmont Health Center.

Wendy Mak, D.O.

Dr. Mak completed her Doctorate of Osteophathic Medicine at the Western University of Health Sciences in 2015. She completed her residency program at the Dignity Health Northridge Family Medicine in 2018. Dr. Mak is certified by the American Board of Family Medicine.  She is comfortable providing care for newborn patients and seniors. Dr. Mak speaks English, Mandarin, Cantonese, and Spanish.

Tiffany Prachachalerm, D.O.

Dr. Prachachalerm joined APHCV in 2021. She completed her Doctorate of Osteopathic Medicine in 2018 at Touro University of Nevada, College of Osteopathic Medicine. Dr. Prachachalerm is excited to help her patients achieve their health goals. In her free time, some of her hobbies include trying different cultural cuisines, cooking, traveling, and playing golf. Dr. Prachachalerm is fluent in English, Thai, and some Medical Spanish.

Varsha M. Puri, D.O.

Dr. Puri grew up in the Bay Area and went on to medical education at the Chicago College of Osteopathic Medicine. She is Board Certified in Pediatrics and completed her residency at the Children’s Hospital of Orange County. Dr. Puri speaks Spanish and Hindi. Her passion is in the health of all children and connecting with children and their families. Dr. Puri sees patients at El Monte Health Center.

Kai En Tang, D.O.

Dr. Tang completed a Doctorate of Osteopathic Medicine at Midwestern University Arizona College of Osteopathic Medicine.  She is trained in pediatrics, geriatrics, obstetrics, adult medicine, and in comprehensive care for medical and mental health.  Dr. Tang is fluent in Mandarin, Spanish, and conversational Japanese.

Martin Tolosa, PA-C

Martin completed the Primary Care Physician Assistant Program at the Keck School of Medicine at University of Southern California and earned his Master of Physician Assistant Practice (MPAP) degree in 2014. He is excited to build relationships with his patients and helping them pursue and achieve their health goals. He speaks English and Spanish. Martin enjoys spending time with family, running, and playing basketball. Martin sees patients at Los Feliz Health Center.

Elizabeth Wilkinson, MSN, FNP-BC, RNC-OB, RN

Elizabeth obtained her Masters of Science in Nursing from California State University, Dominguez Hills and is a Board Certified Family Nurse Practitioner. She also attended California Baptist University for her Registered Nurse degree and studied at University of California, Irvine as an undergraduate. Outside of work, Elizabeth competes in synchronized swimming and loves to garden! Elizabeth sees patients at John Marshall High School Health Center.

Phyllis Woo, MPH, WHNP-BC

Phyllis Woo brings a wealth of knowledge and care to APHCV’s women’s health program as a certified women’s health nurse practitioner. She received her Bachelors of Science and a Masters of Science in Nursing from Columbia University and a Masters of Public Health from the University of Southern California. When she is not caring for APHCV patients, Phyllis can be found outdoors hiking or camping and exploring the many museums LA has to offer. Phyllis practices at Los Feliz Health Center.

Bhasker Venkateswaralu, M.D.

Dr. Venkateswaralu is a Board Certified Internal Medicine physician. He completed his Internal Medicine residency at East Tennessee Medical College, Quillen College of Medicine in Jonhson City, Tennessee. He specializes in internal medicine, urgent care, and emergency medicine. Dr. Venkateswaralu sees patients at Los Feliz Health Center.

Manuel Zhu, D.O.

Dr. Zhu is a graduate of UC Berkeley and was trained at Touro University – College of Osteopathic Medicine. He completed a Family Medicine Residency Program at Adena Health System. Dr. Zhu was widely praised in his Residency program for his warmth, compassion, and sense of humor. He is also fluent in Spanish, having been born in Mexico and spoken Spanish as a primary language.
